For your first visit to our office, we recommend you arrive 15 minutes prior to your scheduled appointment time (20 minutes for families) and bring with you the following information:

• Payment for services and/or co-pay

• Dental insurance card or name, phone number & claims address for your insurance carrier.
  
• Name & date of birth for the policy subscriber & patient.
• Social Security number for the policy subscriber & patient.
• Name of subscriber's employer (if applicable).
• Any current x-rays from your previous dental provider*.

• List of current medications & name & phone for your physician.

For your convenience, we accept the following forms of payment: 

• Cash
• Check
• Visa
• Master Card
• Discover
• American Express and CareCredit balance financing.

CareCredit   is third party balance financing for dental services.  They offer up to 24 months FREE financing.  Apply in our office or use the above link to apply online...it's easy and affordable!
